if its rwd, you have to take the front end off, but then its really accessible. get the chain kit off ebay. also worth checking that the egr valve isn't stuck open, it gives all the same symptoms but not really the rattly top end.

worst bit is the timing chain cover is 'glued' on. ford butcher it off and glue a new one on but if you're careful with a sharp scraper and a knife it'll come off fine. the tensioner and slippers are usually all worn out along with a stretched chain so replace the lot. you'll be amazes how quiet the engine is afterwards as the chain was so rattly before (usually for some time).

Had a couple of these duratorques similar symptoms ended up changing the fuel pump on both one was a 2002 other a 2003 apparently notorious for it other thing that sometimes goes is the flywheel sensor which is a cheap fix but pumps run at about 5-600 quid as streetfighter say's get a code reader plugged in see what fault codes it throws up, don't know what was wrong with the old banana engine, good luck!

What was wrong with a banana was once ford sold you the van, you never needed to take it back to them or buy any parts for it. hell, any idiot could repair a broken cambelt at the side of the road without even being recovered. worst case you might need a few pushrods.
